PANWTF License Plate Application

Instructions

  • Determine what vehicle you would like a PANWTF license plate for and if you want a standard plate (next number available) or a personalized plate

  • Click the button below that corresponds with the type of plate that you want to apply for

  • For standard plates:

    • Please complete sections A and E on the form

    • Write out two checks

      • Check 1 made out to the Commonwealth of Pennsylvania for $34

      • Check 2 made out to the PANWTF for $50

  • For personalized plates:

    • Please complete sections A, D, and E on the form

    • Write out two checks

      • Check 1 made out to the Commonwealth of Pennsylvania for $170

      • Check 2 made out to the PANWTF for $50

  • Mail completed form(s) and checks to Joe Walton at 119 Waltons Lane, Mount Pleasant, PA 15666

Standard license plates will start with number WT2000

Please do not try personalizing your plate as 00001, 00002, etc. They will not be accepted by PennDot.
